The Longevity Measure That Could Change How We Think About Aging

Forget the number of candles on your birthday. Researchers are increasingly interested in something more practical: how well you can move, think, sense, adapt, and maintain your independence.

For decades, aging has largely been measured by a familiar list of problems β€” high blood pressure, diabetes, heart disease, arthritis, memory loss, and other chronic conditions. But researchers studying longevity are increasingly asking a different question. Instead of just asking “what diseases does this person have?”, what if doctors also asked: what is this person still capable of doing?

That idea sits at the center of a concept called intrinsic capacity. The term isn’t exactly a household phrase, but it’s becoming increasingly important in research on healthy aging. The World Health Organization defines intrinsic capacity as the combination of a person’s physical and mental capacities that support functioning and healthy aging. The concept could eventually change how doctors monitor aging β€” and how researchers judge whether treatments actually help people stay healthy for longer.

Aging Is More Than a List of Diseases

Two people can be the same age and have completely different levels of health. One 75-year-old might walk quickly, remember appointments, see and hear well, stay socially engaged, and bounce back easily after physical stress. Another person the same age might struggle with mobility, fatigue, memory, or daily activities. Their medical diagnoses alone may not fully capture that gap.

That’s why intrinsic capacity focuses on function instead. WHO’s healthy-aging framework emphasizes maintaining the functional abilities that let people do the things that matter to them, rather than defining healthy aging simply as the absence of disease. That’s a meaningful shift β€” the goal of longevity research isn’t necessarily to make people live longer while spending more of those years sick. It’s to increase the number of years people can stay capable and independent.

Five Parts of the Aging Process

Researchers generally assess intrinsic capacity across five major areas.

Cognition covers the ability to remember, concentrate, process information, and make decisions. Cognitive changes tend to appear gradually, and tracking them over time can offer information a single diagnosis simply can’t.

Locomotion refers to movement and physical function. Walking speed, balance, the ability to stand from a chair, and measures of physical strength all offer clues about how well someone is maintaining physical capacity.

Sensory capacity β€” vision and hearing β€” is fundamental to independence. Declining sensory ability can affect communication, mobility, and a person’s ability to interact safely with their environment.

Psychological capacity is part of healthy aging too. Mood, motivation, emotional well-being, and psychological resilience all shape how people function as they grow older.

Vitality broadly captures the body’s ability to maintain energy and withstand physical stress.

Together, these five domains paint a far more complete picture than any single blood test or diagnosis ever could.

Why Walking Speed and Grip Strength Matter

Some of the most useful measures of aging turn out to be surprisingly simple. A clinician doesn’t necessarily need an expensive machine to learn something meaningful about physical capacity β€” walking speed and muscle strength alone can offer real information about future functional ability. Research has found that weaker grip strength in midlife was associated with a higher risk of later functional limitations and disability.

That doesn’t mean a weak handshake predicts exactly how someone will age. It illustrates a broader principle: physical reserve matters. Someone with greater strength and mobility likely has more capacity to withstand illness, inactivity, or other physical setbacks before losing their independence.

The Important Part Is the Trend

A single measurement tells researchers something. Repeated measurements tell them much more. Picture someone whose walking speed, strength, or cognitive performance stays relatively stable for several years, versus someone else whose numbers gradually slide. Even if both people are still functioning normally today, that second pattern could be an early warning that something is changing beneath the surface.

That’s part of why researchers are interested in intrinsic capacity as a longitudinal measure rather than a one-time snapshot. WHO’s approach to healthy aging emphasizes catching declines in intrinsic capacity early and building personalized interventions to preserve functional ability. The goal isn’t simply to label someone “healthy” or “unhealthy” β€” it’s to catch changes early enough to actually do something about them.

Could This Help Find Longevity Treatments?

This is where intrinsic capacity becomes especially important to aging researchers. Testing whether a drug actually slows aging is an enormous scientific problem. If researchers want to prove a treatment helps people live longer, they could theoretically have to follow participants for decades β€” expensive, slow, and difficult in practice. So researchers need measurable indicators that can shift over a much shorter time frame.

Intrinsic capacity is being investigated as one possible way to measure whether an intervention is preserving or improving healthspan. The U.S. Advanced Research Projects Agency for Health (ARPA-H) has launched its PROSPR program specifically to develop tools and trials that can identify early changes associated with aging and evaluate interventions designed to extend healthspan, and its researchers are exploring how intrinsic capacity could eventually be used as a clinical endpoint in future aging research. That could have major implications for how longevity treatments get developed and tested down the line.

But a Score Is Not a Magic Number

There’s an important limitation here. Intrinsic capacity isn’t currently a simple universal test you can walk into a doctor’s office and request the way you would a cholesterol check. Different components require different assessments, and researchers are still working out how best to measure and combine them.

The scientific community also still has to determine whether improving someone’s intrinsic-capacity score actually leads to a longer life or better long-term health β€” a measure can be associated with healthy aging without proving that changing the measure causes better outcomes. Researchers are still testing that question, not answering it.

The Goal Is Healthspan, Not Just Lifespan

The distinction between lifespan and healthspan is becoming increasingly important. Lifespan is how long you live. Healthspan is how long you remain in relatively good health and functional condition. Modern medicine has gotten remarkably good at keeping people alive with chronic conditions β€” the next challenge is making those extra years healthier ones.

ARPA-H describes this as a major gap: life expectancy has climbed, but many older adults still spend substantial time dealing with chronic disease and functional limitations. Intrinsic capacity fits naturally into that picture, because it asks a more concrete question β€” are the abilities that make independent living possible actually being preserved?

What Can People Do About It?

Intrinsic capacity isn’t a prescription for one particular supplement, diet, or longevity drug, and there’s no proven method for guaranteeing someone maintains a high intrinsic-capacity level throughout life. But the framework does reinforce several familiar principles of healthy aging: regular physical activity helps preserve mobility and strength, mental stimulation supports cognitive health, managing medical conditions prevents them from unnecessarily accelerating functional decline, and maintaining social connection and psychological well-being matters too.

The important point is that aging isn’t simply something that happens to the body passively. Some aspects of functional decline can potentially be delayed, prevented, or managed β€” and WHO specifically emphasizes prevention and early intervention as ways to maintain intrinsic capacity and delay decline.

The Future of Aging Medicine May Look Different

If researchers can validate intrinsic capacity as a reliable measure, doctors may eventually monitor aging in a fairly different way. Instead of simply recording diseases, medications, and lab results, a medical record could also track changes in mobility, cognition, sensory ability, psychological well-being, and vitality over time.

A gradual decline could trigger earlier intervention. A successful treatment might be judged partly by whether it helps preserve functional capacity. And longevity research could start focusing less on the abstract goal of adding years to life, and more on the practical goal of adding capable years.

The Most Important Number May Not Be Your Age

Your chronological age is easy to measure, but it tells us surprisingly little about what a given person can actually do. Two people born in the same year can have dramatically different physical and mental capacities β€” which is exactly why intrinsic capacity is drawing attention. It offers a way of thinking about aging that’s less about the calendar and more about function.

The concept is still being studied, and researchers haven’t yet proved that using intrinsic capacity to guide interventions will actually extend human life. But the underlying idea is a powerful one: healthy aging may be better understood by measuring what people can still do, not simply by counting how many years they’ve lived. And that shift could eventually change what doctors, scientists, and the rest of us mean when we talk about living longer.
